Customers who migrate to the Premium Plan described will forfeit the right to participate in any former Premium Plan/Premium Prepaid offers. If your Monthly Re-Boost® fails, your monthly service will be converted to regular Basic Pay As You Go service. If you want to re-subscribe to the Premium Plan, you will need to reauthorize and you will only be eligible for the plans described. Customers subscribing to Premium Plan prior to 9/6/06 will remain on the former Premium Plan. Re-Boost Monthly Payments: Money can be added to the Prepaid Credit Balance through any existing Re-Boost channel for customers electing to pay for the Premium Plan. Customer is responsible for maintaining account balance to cover service plan costs, the cost of extra services (e.g., ringtones), and costs associated with exceeding your plan minutes. Credit Card (Automatic) Monthly Payments: Valid major credit card required. Only one (1) credit card may be registered for any Boost telephone number; limit of one (1) Boost telephone number per credit card. Requires online or handset authorization of the recurring Monthly Re-Boost Access Fee and an automatic $15 Auto Re-Boost transaction when the Prepaid Credit Balance is $5 or less. Limit of 7 Auto Re-Boost transactions per 28 day period. Re-Boost Cards cannot be applied when using this payment method.
